Notice Description

1. The subject of the contract is the supply of antiretroviral drugs with delivery to the deposit warehouse: Part 1: Doravirine/Lamivudine/Tenofovir Disoproxil |100/300/245mg |tablet30 Part 2: Raltegravir |400mgtablet60 Part 3: Raltegravir |600mgtablet60 Part 4: Doravirine |100mgtablet30 According to the description of the subject of the contract specified in the specification of the terms of the contract (SWZ) and on the conditions specified in the planned provisions of the contract (PPU), which constitute an attachment to SWZ.

Przedmiotem zamówienia jest dostawa leków antyretrowirusowych z dostawą do magazynu depozytowego: Część 1: Doravirine/Lamivudine/Tenofovir Disoproxil | 100/300/245mg | tabletka | 30 Część 2: Raltegravir | 400mg | tabletka | 60 Część 3: Raltegravir | 600mg | tabletka | 60 Część 4: Doravirine | 100mg | tabletka | 30 Zgodnie z opisem przedmiotu zamówienia określonym w specyfikacji warunków zamówienia (SWZ) oraz na warunkach określonych w projektowanych postanowieniach umowy (PPU), które stanowią załącznik do SWZ.

    The contracting authority requires that the Contractor have a valid entry in the relevant register of manufacturers or importers/exporters of a medicinal product or a valid entry in the register of pharmaceutical wholesalers.
    In order to confirm the fulfillment of the conditions for participation in proceedings or selection criteria confirming the rights to conduct a specific business or professional activity in accordance with § 7 para.1 point 2 of the Regulation of the Minister of Development, Labor and Technology of December 23, 2020 on subjective means of evidence and other documents or statements that the contracting authority may demand from the Contractor (hereinafter referred to as the “Regulation on subjective means of evidence”) The Ordering Party requests:An important, current permit for the production or import of a medicinal product (in the case of a contractor having its registered office or place of residence outside the territory of the Republic of Poland – equivalent document) or a valid, current permission to conduct pharmaceutical wholesalers
    AboveThe documents must be valid as at the date of their submission and confirm that the contractor fulfills the condition of participation in the procedure for the day of submitting the bids.
    In the case of contractors jointly applying for the order, the above condition must be met in full by one of the contractors jointly applying for the award of the contract.
